For a puppy traveling from Utah to Alabama, review four separate parts of the file: official entry paperwork, the puppy’s dated care records, the parents’ screening records and your written purchase terms. Each answers a different question. A travel certificate alone does not establish lifelong health or replace the rest of that review.

Check Alabama’s accepted entry document
Alabama’s published dog-entry guidance requires an official Certificate of Veterinary Inspection. Its allowance for puppies under three months to enter without rabies vaccination is not a certificate exemption. Ask the issuing veterinarian which requirements apply to the puppy’s age and movement.
Alabama does not accept APHIS VS Form 7001 for interstate small-animal entry. Ask the issuing veterinarian to use an accepted certificate and verify the current state instructions. A familiar-looking generic form is not enough.
Recheck current conditions for the actual origin
The same ADAI requirements page carries New World Screwworm updates tied to animal origin. Ask the issuing veterinarian to confirm applicable additional conditions and certificate validity for this journey. A generic ten- or thirty-day window should not replace that check.
Resolve document requirements before departure and revisit them if the date or arrangement changes. If a carrier is involved, confirm its separate acceptance rules. Do not assume an airline accepting a form makes that form valid for Alabama entry.
Match the document to the puppy and trip
- Compare birth date, sex, breed or mix, color and markings across the puppy’s records.
- If a microchip or other identifier is present, confirm which identifier ties the records to this puppy.
- Check the sender, receiving adult, origin and destination against the agreed arrangement.
- Review examination, issue and planned movement dates together with the issuing veterinarian.
- Read the veterinarian’s name, signature and certificate details; clarify missing or inconsistent entries before departure.
Keep readable copies and ask who will provide the required original or accepted electronic document. If something differs, request a clear correction from the appropriate issuer rather than changing a certificate yourself.
Keep the puppy’s care history separate
Ask for dated examination findings, vaccination and deworming records, disclosed concerns, current food and the individual go-home instructions. Bring these to your Alabama veterinarian. That veterinarian should decide the next care steps from the actual record and examination; this checklist supplies no fixed vaccine or medication schedule.
A wellness note, vaccine list and official travel certificate serve different purposes. Ask which document you have and what remains to be confirmed. A reassuring label such as “healthy” should not replace readable records and specific answers.
Review both parents’ records and the written terms
Read Sage Puppies’ health-testing approach, then request reports that identify the specific parents, test types, dates and results. Ask how each result was considered for the pairing. A puppy’s travel exam does not answer those parent-screening questions.
Review the actual health guarantee and your signed agreement for any examination or notification deadline. Arrange your local veterinarian before homecoming and ask how to report a concern. Preserve exact written terms instead of relying on a general article summary.
Build a record file you can use after arrival
Keep your copies accessible for travel and local veterinary care. ADPH’s pet emergency checklist recommends waterproof vaccination, medical and veterinarian records. Put the puppy’s identifying details and current contact information with that file.
If a clinic needs to clarify an entry, use its independently verified public contact details and the permissions required for it to discuss the record. Keep private records within the family, program and professionals involved in the puppy’s care.
