# Wisconsin Puppy Records and Support: What to Confirm Before Homecoming

Before a puppy comes home to Wisconsin, put the actual records, written agreement and receiving plan in one place. Knowing which document answers which question helps you avoid confusing a veterinary examination with parent screening, a health warranty or a promise of ongoing support.

Sage Puppies is Cassie Milar’s South Jordan, Utah program, offering Havanese, Havapoo and Cavapoo puppies. Check the [current profiles](https://sagepuppies.com/available-puppies/)
 and [pricing and reservation information](https://sagepuppies.com/pricing-reservation/)
. General purchase pricing is $3,500–$4,500. Confirm the individual price; travel, routine care and guardian arrangements are separate.

## Build a file for the puppy you are actually considering

Request the puppy’s identifying details, examination record, dated vaccination and deworming history, current food and feeding routine. Ask what has been completed, what remains due and when the documents will be available. Share the file with the Wisconsin veterinarian who will continue care.

For parent screening, request identifiable records for the actual pairing, including the evaluation, date and result. Ask your veterinarian what each record can tell you and what remains uncertain. Sage’s [health-testing approach](https://sagepuppies.com/about/health-testing/)
explains the program’s direction; an overview is not proof that every possible test occurred or every result was normal. An inherited-risk assessment and a puppy examination answer different questions.

## Confirm Wisconsin entry paperwork for this journey

[Wisconsin DATCP’s household-pet entry guidance](https://datcp.wi.gov/Pages/Programs_Services/PetMovement.aspx)
requires an accepted certificate of veterinary inspection, or CVI. It calls for actual origin and destination addresses, animal details and movement purpose. Dogs five months and older need current veterinarian-administered rabies vaccination documented as specified. Ask the accredited veterinarian to verify the accepted form and applicable dates; do not complete or alter official veterinary statements yourself.

Also check [DATCP’s current New World Screwworm movement guidance](https://datcp.wi.gov/Pages/NewWorldScrewworm.aspx)
. Additional conditions depend on affected states and active zones. Have the veterinarian and DATCP review the puppy’s actual history, origin, transit and movement date before committing, and again if those details change. This guide does not establish a state’s disease status, a completed permit or the puppy’s travel eligibility.

## Read written terms separately from veterinary documents

DATCP’s [CVI questions and answers](https://datcp.wi.gov/Pages/Programs_Services/DogSellerCVIFAQ.aspx)
distinguish the veterinary document from a future-health guarantee. Intrastate dog-seller requirements and interstate entry requirements also have different scopes. Do not assume a document used for one transaction answers every question about another journey.

Read the current purchase agreement for covered conditions, exclusions, buyer duties, any required veterinary-exam deadline and the stated remedy. Ask who pays which expenses and what documentation is needed if a concern arises. These are questions to resolve in the actual terms, not a list of promises Sage makes. Keep the agreement and veterinary file together without treating them as interchangeable.

## Agree on who answers which questions

Ask Cassie which channel to use for routine transition questions, what support is included and when a response can reasonably be expected. Write down the food, sleep and handling routine so different caregivers can follow it. Do not build the first week around an assumed response time.

Arrange your local veterinary relationship and an after-hours medical contact before arrival. Medical or urgent concerns need veterinary attention rather than waiting for a program message. Discuss the puppy’s examination and follow-up plan directly with that veterinarian. A supportive placement conversation does not establish round-the-clock availability, remote diagnosis or lifetime services.

## Prepare the receiving home and a backup caregiver

Set up a quiet sleeping space, the agreed food and a practical potty route. In winter, determine who clears a shared entrance and what cleanup is needed after slush or deicer exposure. Keep the arrival evening simple enough to notice eating, resting and settling, instead of adding a crowd of visitors.

[ReadyWisconsin’s pet-planning guidance](https://readywisconsin.wi.gov/planning-for-pets/)
recommends a backup caregiver, an emergency kit and a place confirmed to accept pets if evacuation is necessary. Make the plan usable: tell that caregiver where the records and supplies are, how to contact you and which veterinarian to call. Confirm accommodation rules rather than assuming a shelter or hotel admits dogs.

## Review the plan before application and travel

Read the [Wisconsin household guide](https://sagepuppies.com/9-places-to-find-puppies-for-sale-in-wisconsin/)
 and [listing checklist](https://sagepuppies.com/how-to-spot-a-safe-wisconsin-puppy-listing-online/)
 together. Review [the placement process](https://sagepuppies.com/our-program/how-it-works/)
 and [delivery and pickup information](https://sagepuppies.com/delivery-pickup/)
. Confirm actual readiness, travel responsibilities, costs and how a delay changes the plan before booking.

[511 Wisconsin](https://511wi.gov/)
provides road information; separately check the receiving property’s access and agree on a weather-change contact. Budget for ongoing veterinary care, grooming and caregiver help as well as the purchase. When the records and household plan are ready for discussion, [apply with your details](https://sagepuppies.com/application/)
. Approval, a reservation and travel arrangements require individual confirmation.
