# Eight Puppy Records to Review Before an Illinois Homecoming

Before bringing a puppy to Illinois, match the paperwork to the puppy, the people involved and the actual journey. The eight categories below are practical review points. Several may share one document, and their legal requirements depend on the transaction and circumstances.

Sage Puppies is Cassie Milar’s South Jordan, Utah program for Havanese, Havapoo and Cavapoo puppies. Use [the Illinois buyer guide](https://sagepuppies.com/puppies-for-sale-in-illinois-a-buyers-guide/)
to choose a match and [the homecoming preparation guide](https://sagepuppies.com/illinois-puppy-pickup-guide-what-new-owners-should-bring/)
for the household setup. Ask Sage and the relevant professionals which records apply to your puppy.

Sage’s general puppy purchase range is **$3,500–$4,500**. Each listing gives the puppy’s exact price; guardian arrangements and travel costs are separate. Review [Pricing & Reservation](https://sagepuppies.com/pricing-reservation/)
 and [How It Works](https://sagepuppies.com/our-program/how-it-works/)
 before committing.

## 1. Puppy identity and source details

Check the documented birth date, sex, breed or parent backgrounds, identifying description and name of the person or program that raised the puppy. Compare these details across the agreement, care history and travel records. Ask about an unexplained mismatch rather than guessing that two names refer to the same puppy.

## 2. Dated veterinary care and treatment history

Request the individual puppy’s veterinary care, vaccination and treatment history, with dates and provider information where available. Share it with your own veterinarian to plan follow-up. A printed schedule or a “vet checked” label cannot establish what this puppy actually received.

## 3. Relevant parent health records

Ask which parents the records identify, which tests or evaluations were performed and what the findings and limitations mean. Use [Sage’s health-testing information](https://sagepuppies.com/about/health-testing/)
to prepare questions. Parent results inform a review; they are not a guarantee of every future health outcome.

## 4. The written purchase agreement

Read the actual agreement before signing. Check the parties, puppy description, purchase amount, responsibilities and proposed handoff. Ask for clarification about a term you do not understand. Keep the signed version and any agreed changes together so you can refer to the same text later.

## 5. Reservation terms and payment receipts

Review [the current reservation information](https://sagepuppies.com/pricing-reservation/)
and the written terms for your arrangement. Confirm what a payment reserves, due dates and the actual provisions for a change or cancellation. Keep receipts identifying the payment and its purpose. Do not infer refund rights or included travel from the purchase range.

## 6. The written health-guarantee conditions

Read [Sage’s health-guarantee terms](https://sagepuppies.com/health-guarantee/)
and the version included with your agreement. Note the relevant conditions, steps, time limits and contact route. Ask how a concern should be raised. An article summary should not replace the actual terms or professional advice about a puppy’s health.

## 7. The documents for the actual journey

Illinois’s [current dog-entry guidance](https://agr.illinois.gov/animals/animalhealth/animal-import.html)
calls for a CVI issued within 30 days of entry with puppy and destination details; dogs 16 weeks and older need rabies vaccination with the date and manufacturer information recorded. Arrival at an Animal Welfare Licensee also requires a permit. The page says Illinois does not accept APHIS 7001 and announces electronic CVIs for imports from January 1, 2027. Confirm the currently accepted format and all applicable checks with the issuing veterinarian for your date and arrangement.

The [current animal-health alerts](https://agr.illinois.gov/animals/animalhealth/alerts-and-important-animal-health-information.html)
link [the formal NWS emergency rule](https://agr.illinois.gov/content/dam/soi/en/web/agr/animals/animalhealth/documents/NWSW%20Emergency%20Rules_8-85CD-E%206.29.26.pdf)
. For companion animals from a state containing an infested zone, it adds permit and certification checks and distinguishes origins inside and outside that zone. Outside the zone, it specifies a CVI with NWS certification completed no more than five days before movement; additional documents apply within an infested zone. Have the issuing veterinarian and state authority resolve the applicable category and requirements; do not assume Utah’s status or eligibility from a past trip.

## 8. Current care instructions and identification follow-up

Ask for the puppy’s current food, feeding and care routine and the contact for questions. If a microchip is present, check the recorded number and what registration or contact update you need to complete. Confirm what is provided for this puppy rather than assuming every program includes the same chip, supplies or packet.

## Use the form that fits the actual transaction

The [Illinois Animal Welfare resource](https://agr.illinois.gov/animals/animalhealth/animal-welfare.html)
links regulated seller types and licensing information. The state-hosted [pet-shop disclosure form](https://agr.illinois.gov/content/dam/soi/en/web/agr/animals/animalhealth/documents/petdisclosure.pdf)
illustrates identifying and care fields for that transaction type. It is not automatically Sage’s required form or an eight-document legal mandate; ask the authority what applies to the parties and placement.

Keep readable copies available to the adults responsible for the journey and to your veterinarian. Then use [the Illinois preparation guide](https://sagepuppies.com/illinois-puppy-pickup-guide-what-new-owners-should-bring/)
to set up care at home, [check current puppies](https://sagepuppies.com/available-puppies/)
and [apply when your questions and plans are ready](https://sagepuppies.com/application/)
.
